Job Responsibilities

• Provide skilled nursing care in patients' homes
• Complete patient assessments and develop care plans
• Administer medications and treatments
• Perform wound care and IV therapy as ordered
• Educate patients and families on disease management and home care
• Coordinate care with physicians and interdisciplinary teams
• Maintain accurate clinical documentation
  Requirements
• Active Illinois RN License
• Current BLS Certification
• Minimum 1 year of professional nursing experience within the last 3 years in an acute care setting
• Home Health experience preferred
• IV Therapy, ER, or Oncology experience preferred

• Cardiac or Wound Care certification preferred

  Important Notes
• No local candidates within 50 miles
• Mileage between patient visits is reimbursed
• First-time travelers accepted
