Case Manager

Role Description This full-time, on-site Case Manager role is based at Riverbend in Lowell, MA. The Case Manager coordinates and delivers case management services for individuals with substance use and mental health needs, including intake assessments, care planning, and ongoing client support. Daily responsibilities include developing and updating individualized service plans, monitoring progress, advocating for clients, and linking them to internal programs and external community resources such as housing, employment, and healthcare. The Case Manager maintains accurate documentation, collaborates with clinical and support staff, participates in team meetings, and ensures services align with organizational policies and applicable regulations. The role also involves crisis support within scope of practice, providing education on recovery resources, and helping clients navigate complex service systems. Qualifications Demonstrated skills in case management, care coordination, and advocacy for individuals with behavioral health or substance use needs. Ability to conduct client assessments, develop individualized service plans, and monitor outcomes using a structured, goal-oriented approach. Strong interpersonal, communication, and active listening skills to build trust and collaborate effectively with clients, families, and multidisciplinary teams. Competence in documentation, recordkeeping, and basic computer skills, including use of electronic health records or similar systems. Knowledge of community resources, social services, and recovery-oriented supports in behavioral health and substance use treatment. Capacity to manage time, prioritize tasks, and maintain professional boundaries in a fast-paced, on-site environment. Experience working with diverse populations and commitment to culturally responsive, trauma-informed, and non-judgmental care. Relevant education such as a bachelor’s degree in social work, psychology, human services, or related field; equivalent experience in case management or behavioral health may be considered.
