Registered Nurse (RN) - Case Manager

We are currently seeking a skilled and dedicated Registered Nurse to join our team as a Case Manager. The successful candidate will be responsible for performing care coordination and discharge planning within an acute care setting.

Key Responsibilities

Provide comprehensive care coordination for patients within the assigned unit.
Conduct thorough discharge planning to ensure safe transitions for patients.
Collaborate with healthcare teams to optimize patient care outcomes.
Maintain accurate records and documentation of patient interactions and care plans.
Participate in departmental initiatives and contribute to performance improvement activities.

Qualifications

Active California state nursing license; pending licenses are accepted but must be valid upon starting date.
Minimum of 2 years of acute medical care management experience.
Ability to work one full weekend per 4 weeks, subject to department needs.
No more than 7 days of requested time off.
First-time travelers must undergo screening.

Work Schedule

Shift: Day shift, 5 days a week (08:00 AM - 04:30 PM)
No floating expectation required.
