Care Manager - Behavioral Health in hospital setting
- 2+ years of recent Behavioral Health experience
- 5+ years minimum of clinical experience as a Registered Nurse
- PRI & Screen Certification
- Strong Behavioral Health clinical review and case management experience
- Care Management Certification, leadership experience, telephonic clinical reviews, interdisciplinary team collaboration
- Experience with Care Coordination, Utilization review and discharge planning
Key Responsibilities
- Conduct telephonic clinical reviews
- Complete PRI & Screen assessments
- Participate in interdisciplinary meetings
- Coordinate Behavioral Health patient care
- Manage Behavioral Health caseloads
Preferred Skills
- Care Management Certification, leadership experience, telephonic clinical reviews, interdisciplinary team collaboration
- BSN-qualified RN with recent Behavioral Health case management experience, strong communication skills, organized, collaborative, confident in multidisciplinary settings, knowledgeable in Behavioral Health diagnoses and medications
Licensure & Certifications
NY state RN license
BLS - American Heart Association
PRI & Screen Certification
Education
Associates Degree (ASN, ADN) from accredited nursing program - REQUIRED
Bachelors Degree - BSN preferred
**PRI and Screen certification within three months of employment is required.
