Description

American Traveler is hiring an experienced RN for a Hospice/Palliative Care position in a home health setting, requiring Homecare Homebase (HCHB) experience and prior hospice background.

Details

• Home health / hospice care setting serving patients across Nelson, Spencer, and southern Bullitt counties in Kentucky
• Patient population includes both pediatric and adult hospice patients
• Monday–Friday, 8:00 AM–5:00 PM schedule with 8-hour shifts
• On-call required one weeknight per week and every 3rd weekend (includes both scheduled and on-call visits)
• Clinicians are responsible for calling patients directly to schedule visit times
• Daily responsibilities include Start of Care assessments, recertifications, and daily visits
• Charting performed in Homecare Homebase (HCHB) — experience is required
• No floating required

Requirements

• Active KY RN license or Compact (NLC) RN license — must be active at time of consideration; pending licenses will not be accepted
• Current BLS certification required — pending certifications not accepted
• Minimum 1 year of RN experience required
• Prior hospice experience required — no exceptions
• Homecare Homebase (HCHB) EMR experience required — no exceptions
• Home health background required
• Valid driver's license and personal auto insurance required
• 2 professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked — must include dates of employment and eligibility for rehire
• A copy of your driver's license is required for consideration

Additional Information

• Orientation is approximately 12 hours
• Home health/hospice aides are available to support the care team
• No holiday coverage required
• Any color scrubs are acceptable
• Mileage is not reimbursable for this position
• 13-week contract with a 40-hour work week
