LPN – Home HealthSchedule: 8 hoursAssignment Details:Contract Length: 13 weeksGuaranteed Hours: 40Requirements:Current nursing license in the practicing stateValid driver's licenseCurrent CPR certificationTwo years of experience as an LPN in a clinical setting, preferably in a home health or hospice settingProficiency in using electronic medical charting and time-keeping software (Homecare Homebase)Roles & Responsibilities:Provide nursing services under the direction of a registered nurse, assessing patients' physical, psycho-social, and environmental needs.Assist in planning, coordinating, and implementing patient care in collaboration with the RN.Communicate effectively with patients, families, and healthcare professionals and document care outcomes appropriately.Educate patients and families on self-care techniques and assess their responses to care.Maintain infection control measures to ensure safety for both staff and patients.Assist with specialized duties related to end-of-life care for hospice patients.Support with patient admissions, discharges, and transfers.Compliance / Notes:Must participate in annual in-service and performance improvement programs.Reliable transportation and current auto liability insurance coverage are required.Must read, write, and speak fluent English.Client Details:City: RaleighState: NC
