Pharmacist

This position is assigned at Hampton VA Medical Center - Western Branch Community Based Outpatient Clinic (WB CBOC). The Outpatient Clinical Pharmacist supports the general operations of pharmacy including prescription/order processing, outpatient dispensing, patient consultation, and perform all pharmacy related activities as assigned and on any shift. Duties include but are not limited to:

• Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
• Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and/or in writing.
• Serves as a drug information resource by providing up-to-date drug information to prescribers, other health care professionals, patients, and caregivers.
• Recommends formulary alternatives and completes non-formulary and restricted medication consult as assigned.
• Manages various actions, as appropriate, for medication orders to include flagging prescriptions, managing view alerts, and generating unassigned discontinued orders using service reject.
• Interpret various VA, Federal (FDA, DEA), other regulatory agency compliance standards (e.g., Joint Commission) Facility and Pharmacy Service policies and be able to apply the same to given situations. Ability to exercise sound judgment in a variety of situations and be able to refer matters of policy making to a supervisor.
• Supervises and directs pharmacy support personnel in the absence of a supervisor. This includes verification of the daily activities of pharmacy technicians.
• Works independently with minimal supervision. Organizes and prioritizes work assignments.
• Ensures pharmacy services are provided in a timely and efficient manner.
• Able to multi-task e.g., able to continue to process orders and answer questions from technical staff and/or patients (via phone or in-person).
• Trains professional staff including pharmacy, medical staff (including fellows and residents) and students.
• Interfaces with other healthcare providers and administrators to ensure compliance with established pharmacy and medical center policies and procedures.
• Responsible for resolving a variety of questions posed by physicians, nurses and co-workers related to drugs, pharmacy services and departmental policies and procedures.
• Assists with the development of quality and safety improvement initiatives and the collection and analysis of data. Examples include but are not limited to order sets, protocols, monographs, and participation in committees or workgroups.
• Maintains competence required for current position including required TMS training, and BLS certification.
• Properly operates all equipment in the assigned area(s) in an efficient manner and assist with troubleshooting automated dispensing
• Maintains adequate drug stock, inspects drug storage areas, supervises pre- packing and the operation of automated dispensing equipment.
• May be assigned to assist with duties or other work areas in pharmacy service to help fulfill our mission and abide by our organization's values.

Work Schedule: Monday - Friday, Various 8 hours. May be required to work rotating weekends and holidays. Tours may be adjusted to meet medical center and patient care needs.
