As part of our Hospice Center services, you will engage with clients during critical times, emphasizing compassionate care and emotional support. Key Responsibilities Provide expert therapeutic support and counseling to clients experiencing a wide array of emotional, behavioral, and psychosocial issues. Actively engage with clients through virtual sessions, utilizing telehealth technology to ensure continuity of care. Serve as an advocate, helping clients navigate and access community resources, social services, and additional support systems that improve their quality of life. Conduct thorough assessments and develop individualized treatment plans, regularly reviewing progress and adjusting strategies as necessary. Maintain all client records with accuracy and confidentiality, adhering to HIPAA regulations and ethical standards in social work practice. Collaborate effectively with psychiatrists, psychologists, nurses, and other healthcare professionals, participating in case consultations to optimize care delivery. Participate in community outreach initiatives that enhance referral resources and strengthen partnerships within the community. Educate clients on essential coping skills, communication techniques, and self-care practices to empower their journey toward mental wellness. Patient Population / Client Focus The licensed clinical social worker will focus on a diverse client population, including adults who may be experiencing significant life transitions, grief, depression, anxiety, and other mental health conditions. Many clients are dealing with the challenges associated with chronic illnesses or terminal diagnoses, necessitating a compassionate and tailored approach to therapy. This role involves not just treating symptoms but addressing the broader psychosocial factors affecting clients' well-being. Qualifications Current certification and licensure as a Licensed Clinical Social Worker (LCSW) in the state of practice. A Master’s degree in Social Work (MSW) or a closely related field is required. A minimum of four years of experience in clinical social work, specifically within an outpatient or related treatment setting. Excellent interpersonal, verbal, and written communication skills, with a strong emphasis on relationship-building. Commitment to maintaining ethical standards and client confidentiality in therapeutic practices. Ability to thrive both independently and as part of an interdisciplinary team, facilitating collaboration across various healthcare domains. Preferred Experience Experience with telehealth delivery of therapy services and familiarity with relevant technologies. Compensation and Benefits This role offers a competitive salary ranging from USD 90,000 to 100,000 per year, commensurate with experience and qualifications.
