Hospice RN Admissions Nurse

Addus Family Company is seeking a Hospice RN Admissions Nurse to join our mission-driven hospice team. In this role, you will complete timely, compassionate admissions assessments in patients’ homes or facilities, verify hospice eligibility, and educate patients and families on the hospice philosophy, services, and plan of care. You will collaborate closely with physicians, IDT members, and referral sources, obtain consents, initiate care plans, and ensure regulatory-compliant documentation. This position offers meaningful patient contact, strong clinical support, and growth opportunities within a nationwide home health and hospice organization.
