We're currently seeking a Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT/FACT) team.

Starting rate of $30.30 hour + mileage reimbursement

Schedule: MondayFriday, 8:00 AM4:30 PM, plus rotating on-call coverage on consecutive days and weekend community coverage.
Location: 6379 Center Drive Norfolk, VA 23502

Position Overview: As an integral member of the interdisciplinary team, the LPN plays a key role in supporting individuals with serious mental illness through a recovery-focused approach. This includes:

Provide recovery-based, person-centered clinical services through individual and group therapy, supporting mental health, daily living skills, and social development.
Develop, implement, and regularly update treatment plans in collaboration with individuals, families, and the interdisciplinary team.
Conduct comprehensive assessments and provide ongoing evaluation of symptoms, treatment responses, and overall progress.
Support overall health needs by providing medication education, monitoring, administration, and addressing physical health concerns.
Act as a liaison and advocate by coordinating with families and community agencies, offering education, crisis support, and connections to resources.

The ideal candidate will possess the following qualifications:

Licensed Practical Nurse (LPN). Active LPN license issued by the Virginia Board of Nursing
2 years of experience providing mental health services to adults with serious mental illness
At least 1 year of experience in medication management
Strong knowledge of recovery principles and recovery-driven care
Valid Virginia driver's license
