Occupational Therapist - Travel Contract

Make a meaningful difference as a travel OT in Phoenix, AZ. We seek a dedicated Occupational Therapist to provide coordinated, patient-centered care in a home health environment. The clinician will plan, implement, and evaluate individualized care plans to help patients develop, regain, or maintain daily living skills and independence.

Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program.
Current, active Arizona Occupational Therapist license required prior to start.
Proof of current CPR certification required.
Current driver's license, reliable transportation, and vehicle insurance required due to home visits.
Minimum two years of experience as an Occupational Therapist in an acute care or rehabilitation setting required.
Experience performing home health evaluations, developing Plans of Care (POC), progress notes, and documenting clinical interventions in a home care environment preferred.
Ability to fabricate and maintain adaptive equipment and hand splints preferred.
Ability to perform clinical assessments of ROM, strength, coordination, functional mobility, ADL performance, and cognitive-perceptual screening preferred.
Ability to work a flexible schedule and travel locally within the service area required.
