Job Description
Join Our Dynamic Healthcare Team as a Case Manager!

Job Description:


- Shift: Day 5x8-Hour (08:00 - 16:30)
- Start Date: ASAP
- Floating Expectation: No
- Patient Ratios: 1:20
- Experience Required:
- Minimum of 2 years in acute care Hospital Case Manager role
- Expertise in Care Coordination and Discharge Planning
- Prior hospital experience is a must!
- First-Timers Accepted: Yes
- Clinical will screen first-time travelers

Requirements:


- California State License: Required (Pending Licenses accepted, must be licensed by start date)
- Weekend Requirements: Weekend rotation to meet department needs
- RTO Restrictions: No more than 7 days

Hospital Highlights:

Key Responsibilities:


- Perform thorough assessments and develop treatment plans.
- Participate in family meetings and coordinate care effectively.
- Work with interdisciplinary teams including Social Services and Transportation.
- Manage care for patients with complex needs, including mental health and substance abuse issues.
- Arrange necessary services such as DME, SNF, and ambulance transport.

Special Equipment:


- Computerized documentation required
- Proficient in Microsoft applications

Traveler Expectations:


- May float between nearby facilities as needed
- Refusal to float may impact assignment duration and guaranteed hours
- Strong commitment to collaboration within a team setting

- Must include two professional references covering at least one year of employment within the last three years.
