Acute Care Hospital Case Manager

Job Description

This is a 13-week contract position for an Acute Care Hospital Case Manager. The role will require performing care coordination and discharge planning for patients within an acute care facility. Candidates must have a solid background in hospital case management with a focus on discharge strategies.

Responsibilities

• Perform care coordination and effective discharge planning for patients.
• Utilize InterQual criteria for assessing patients and developing treatment plans.
• Organize and participate in family meetings to enhance patient care.
• Arrange for durable medical equipment (DME), skilled nursing facilities (SNF), and ambulance transport as needed.
• Support interdisciplinary teams involving social services and transportation.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care hospital case management experience.
• Current California state license required; pending licenses accepted if licensed upon start date.
• Experience in care coordination, discharge planning, and knowledge of CMS guidelines.
• Familiarity with Electronic Health Records, specifically Epic preferred.

Schedule

• Day shift: 5x8 hours (08:00 - 16:30)
• Weekend rotation required, based on department needs.
