Position Overview:

BVHPF is seeking a Physician Assistant or Nurse Practitioner (PA/NP) to serve as a clinical leader within our integrated care team. In this role, you will deliver high-quality primary care to patients navigating substance use recovery, co-occurring mental health conditions, and the social determinants that shape their health.

You will provide clinical oversight, participate in hiring and performance review of clinical support staff, and collaborate with a supervising physician in accordance with California state law. Above all, you will help advance a model of care that is trauma-informed, non-judgmental, and grounded in our organization's commitment to dismantling racism, gender discrimination, and other forms of structural harm, both within our walls and in the broader systems we navigate alongside our patients.

Duties and Responsibilities:

Clinical Care

• Provide comprehensive primary care to BVHPF patients within the scope of credentialed privileges and standardized procedures, using a team-based care model.

• Conduct approximately 20 hours per week of medication assessments, including MAT/MOUD evaluations for patients with opioid and other substance use disorders.

• Coordinate timely follow-up for specialty referrals, laboratory results, and radiology findings, ensuring no patient falls through the cracks.

• Respond promptly to patient requests for information, form completion, and prescription refills.

• Maintain timely, accurate documentation that meets BVHPF charting protocols and California state licensing requirements.

• Participate in rotating on-call after-hours coverage.

Integrated & Trauma-Informed Practice

• Partner closely with mental health staff to strengthen integration between primary care and behavioral health, improving outcomes for patients with co-occurring disorders.

• Apply trauma-informed clinical principles (per NIDA and SAMHSA guidance) in every patient interaction, with particular attention to building rapport and trust with individuals in recovery.

• Support patients with empathy and without stigma, recognizing addiction as a treatable medical condition.

Leadership & Quality Improvement

• Provide clinical supervision and oversight of support staff, including participation in hiring, performance review, and (when necessary) termination decisions.

• Achieve benchmarks defined in the annual quality management plan, including building a continuity panel and meeting productivity standards.

• Act as a change champion in QI initiatives, partnering with colleagues to identify and resolve recurring barriers to care.

• Supervise students and trainees as assigned.

Organizational Engagement

• Represent BVHPF at professional conferences, in-service trainings, and community meetings as directed.

• Lead case presentations and in-service training for clinical staff.

• Complete all annual mandatory training.

• Maintain licensure, DEA registration, and BCLS/CPR certification, and pursue continuing professional education.

Minimum Requirements and Experience:

• Master's degree (MSN) from an accredited school of nursing, OR equivalent PA credentialing.

• Valid California license as an ANP, FNP, or PA, with a furnishing number.

• National certification from the AANP (or equivalent for PA candidates).

• Current DEA Controlled Substances Registration Certificate.

• Current BCLS/CPR certification.

• Demonstrated ability to provide compassionate, non-judgmental care to patients with substance use disorders, and to build trust through every stage of recovery.

• Strong grounding in the ethical and legal principles governing patient confidentiality (including 42 CFR Part 2), informed consent, and the prescribing of controlled substances.

Preferred Qualifications:

• Two or more years of post-graduate experience as an NP or PA in a clinical setting.

• Experience in addiction medicine, substance use treatment, or MAT/MOUD program delivery.

• Working knowledge of methadone treatment protocols, including assessment, monitoring, dosing, and the federal and state regulations governing methadone care.

• Experience in ambulatory care and/or a community health center setting.

• Background in primary care, behavioral health, or integrated care models.

• Knowledge of co-occurring disorders and trauma-informed treatment frameworks.

• Demonstrated cultural humility and experience serving diverse populations, including the LGBTQ+ community, people experiencing homelessness, people with histories of incarceration, and individuals navigating poverty.

• Bilingual fluency in Spanish and English.

• Comfort working within multidisciplinary teams and safety-net care settings.

• Proficiency with electronic medical records and core Microsoft Office applications (Word, Outlook).

· Ability to work effectively in an open office environment surrounded by moderate noise and distractions.

· Frequently required to sit; occasionally walk and stand; travel from the building to other sites.

· Specific vision abilities required by this job include close vision, distance vision, depth perception, and the ability to adjust focus.

· The employee must be able to meet deadlines with time constraints.

· May be asked to occasionally work irregular or extended hours while performing job duties.

· Able to meet required state, federal, local and BVHP standards.

· Live Scan fingerprinting and TB clearance as well as any other medical vaccinations may be required.

· At least two COVID-19 vaccinations (preferred).
