Licensed Practical Nurse

Job Summary In a team relationship with the Registered Nurse, the Licensed Practical Nurse contributes to the nursing process through ongoing systematic data collection, assisting with the health assessment, recording observations, goal identification, and patient health education. Initiate the admission process and assist with the identification of recurrent health problems, symptoms, and behavioral changes of the patient. Collect pertinent data through observation, interview, and other members of the health team and communicate changes in the patient condition to others. Record all data obtained through assessment in the patient’s health record. Assist in the formation of a written plan. Assist in identifying both short and long term, timed, measurable goals with anticipated behavioral change. Assist in setting priorities when planning care to meet patient needs. Assist with patient health care and participate in multidisciplinary care conferences. Count CDS drugs and contraband, if required. Assist physician in minor medical/surgical procedures. Transcribe and post all therapeutic orders generated by the physician. Initiate and document prescribed treatments for nursing interventions in compliance with policy and procedure. Administer routine and/or PRN medications as ordered. Practice universal precautions when providing nursing procedures. Assist in maintaining a neat, clean and safe environment for patients and co‑workers. Keep patient information confidential and respect patients’ right to privacy. Keep health team informed of observations and changing conditions of patients. Maintain bi‑annual proof of active state nursing licensure in the personnel file maintained by the RN Supervisor. Appear neat in appearance and adhere to nursing code policy; wear a white lab coat. Arrive promptly for scheduled shifts and provide a doctor’s excuse if absent as required. Notify the RN Supervisor if becoming ill on duty. Submit requests for vacation or special time‑off in advance of posted scheduled time. Arrange workday exchange in the same pay period with a comparable member of the nursing staff for desired days off if the schedule has already been posted. After agreement is reached, notify the Charge Nurse and/or the RN Supervisor in writing of the desired schedule change agreement. Qualifications Graduate of an accredited Licensed Practical Nurse program with current licensure in the state of employment. Must conform to the security clearance standards of the institution. Functional Demands Ability to pass all required health and other screening tests. May be required to work nights, weekends, and holidays. On‑call duty may be required (available to work on short notice). Provides a positive and professional representation of CFG Health Systems, LLC. Promotes a culture of safety for patients and employees through proper identification, reporting, documentation, and prevention. Maintains competency and knowledge of current standards of practice, trends, and developments in related scope of job role or practice. Adheres to infection control policies and protocols, medication administration and storage procedures, and controlled substance regulations. Participate in ongoing quality improvement activities. Completes required orientation as directed by the facility. Follows facility and OSHA safety rules and procedures while on assignment. Follows facility and CFG Health Systems Occurrence Protocol. Upholds HIPAA and PREA regulations. Punctual and dependable for assigned/confirmed shifts. Maintains security clearance for the assigned facility. Conforms to dress code.
