The RN Supervisor ensures that residents receive safe, high-quality care consistent with the standards of practice, regulatory requirements, and the mission of Constant Care Agency. This position requires strong clinical judgment, supervisory ability, and compassion for the elderly and long-term care population./ph3Primary Responsibilities/h3pClinical Supervision/pulliOversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts./liliEnsure timely and accurate delivery of nursing care in accordance with physicians orders, facility policies, and resident care plans./liliMonitor residents conditions; respond promptly to changes in status and initiate appropriate interventions or physician notifications./liliConduct walking rounds at the beginning and end of each shift; verify continuity of care and accurate reporting./liliOversee medication administration and treatments to ensure safety and compliance./li/ulpStaff Leadership Management/pulliAssign, direct, and evaluate the performance of nursing staff; delegate tasks appropriately based on scope of practice./liliProvide mentorship, education, and on-the-job training for new or less experienced staff./liliEnsure adequate staffing levels; manage call-ins and replace staff as needed in collaboration with the staffing coordinator./liliPromote teamwork, professionalism, and accountability across the care team./li/ulpRegulatory Compliance/pulliEnsure nursing practices comply with New York State DOH, CMS, and facility policies./liliConduct chart audits to confirm accurate, complete documentation of resident care and progress notes./liliParticipate in QA/QI programs, infection control audits, and safety committees./liliPrepare for and participate in DOH inspections and surveys./liliMaintain resident confidentiality and HIPAA compliance at all times./li/ulpEmergency Response/pulliServe as the on-site clinical authority during assigned shifts./liliRespond to medical emergencies, perform immediate assessments, and direct staff response./liliCommunicate critical incidents to the DON or Administrator on call./li/ulpResident Family Relations/pulliFoster compassionate, resident-centered care and ensure residents dignity and rights are upheld./liliCommunicate effectively with residents and families regarding care issues or changes in condition./liliParticipate in care plan meetings when required./li/ulh3Qualifications/h3ullipLicensure: Current, unencumbered Registered Nurse (RN) license in New York State./p/lilipExperience: Minimum 23 years of RN experience, with at least 1 year in a long-term care or skilled nursing facility. Prior supervisory experience strongly preferred./p/lilipEducation: Graduate of an accredited nursing program (BSN preferred)./p/lilipCertifications: BLS required; ACLS preferred./p/lilipKnowledge: Comprehensive understanding of NYS DOH and CMS nursing home regulations. Familiarity with MDS documentation, infection control, fall prevention, and pressure injury protocols./p/lilipSkills: Strong clinical assessment and problem-solving ability. Excellent leadership, delegation, and interpersonal communication skills. Ability to handle emergencies calmly and professionally.
