The Community Health Navigator aims to improve health equity for Medi-Cal members through health education and resource referrals by providing outreach witha focus on preventive health services to prevent disease, disability and other health conditions or their progression; to prolong life; and promote physical and mental health and well-being. CHN’s increase awareness of services, promote preventative care and reduce emergency visits. Qualifications HighSchooldiploma or GED required. Proficient in Microsoft suite (Word, Excel, PowerPoint). Knowledgeofcommunity resources. A valid California driver’s license is required of staff who have regular driving duties and the driving record must meet the requirements of their position and be approved by the agency’s insurance company. EssentialJob Functions Operate an agency or personal vehicle for work-related activities which may include, client visits, meetings, trainings, or other agency-related appointments Ensure compliance with driving requirements, including maintaining a valid driver’s license, current vehicle registration, and proof of insurance as required by agency policy. Responsibilities Utilizepreventionstrategiestopromotestigmaanddiscriminationreduction, suicide prevention, mental health awareness and well-being. Provideaccess,advocacy,resourcesandlinkagetoclientsandtheir families. Interactproductivelywithinamulti-disciplinaryteamandothercommunity organizations. Maintainrecordandtrackingsystemsthatmeetstandardsoftheagency,The Joint Commission and contract. Must implement program goals and objectives to meet and maintain contractual requirements and productivity. Effectivelyusede-escalationskillsandtechniquesforcrisis prevention. ProperlyadministertheC-SSRStohelppreventsuicide Other Attendallrequired meetings and trainings.
