Supervisor - Inpatient Case Management Shea

Responsibilities:

JOB SUMMARY

The Inpatient Case Management Supervisor is accountable and responsible for overseeing daily operations within assigned areas of the department. The position oversees case management staff including RN Case Managers, SW Case Managers, and Case Management Assistants. Responsibilities include monitoring staff activities, performance, and development, as well as evaluating new processes and improvements. The supervisor conducts first‑level problem solving, ensures schedules are complete, resolves scheduling issues, and escalates appropriate matters to senior case management leaders. The supervisor is responsible for ensuring adherence to standard work and removing barriers to completion. The position also participates in interviewing, hiring, onboarding, orientation, and provides input on performance issues within assigned teams.

ESSENTIAL FUNCTIONS

Supervises, in conjunction with Case Management leadership, personnel in performing their duties. Participates in interviewing and hiring recommendations. Assigns and directs duties. Ensures subordinate work complies with departmental standards and administers first‑line discipline as necessary. Observes and documents performance and compliance with established standards in the appraisal process. Participates in short‑term and long‑term staffing determinations. Assists leadership in higher‑level disciplinary processes and develops related Performance Improvement Plans. Reviews patient/customer/employee complaints to identify issues and trends. Investigates further as appropriate and provides recommended resolution. Assists in developing, implementing, and evaluating unit‑specific programs and process improvement initiatives that maintain staff skill levels. Supports and facilitates staff involvement in programs and committees, encouraging a professional environment. Fosters collaborative partnerships among all case management disciplines, medical staff, and related system departments, especially Nursing. Serves as first point of escalation for problem solving and barrier resolution. Ensures standard work adherence and assists with change management. Develops internal processes, identifies improvement needs, monitors initiatives, institutes remedial action, disseminates data to staff, and recommends modifications to standard work using a data‑driven approach. Provides backup case management coverage in the hospital as needed. Performs other duties determined by senior case management leadership to support departmental needs.

EDUCATION

Bachelors Nursing Required or Masters Social Work Required Masters Nursing or other healthcare profession Preferred

EXPERIENCE

2 years of inpatient case management or inpatient medical social work discharge planning with evidence of informal or formal leadership within the team Required

LICENSE AND CERTIFICATIONS

Registered Nurse (RN) – License, or Licensed Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) Required Certified Case Manager – Certification in a related field (e.g., case management certification) Preferred We're all in for your career. #J-18808-Ljbffr HonorHealth
