Job Title: LPN - Long Term Care
Location: Falmouth, MA
Type: Travel Contract | 13 Weeks
Shift & Schedule: Mixed Days & Evenings | 8-Hour Shifts | 40 Hours/Week | Every Other Weekend & Holiday
Position Overview:
Navitas Healthcare, LLC is seeking LPN - Long Term Care for an exciting travel opportunity in Falmouth, MA.
Responsibilities
- Provide direct nursing care to residents according to individualized care plans
- Supervise and support nursing assistants during assigned shifts
- Administer medications and treatments as prescribed
- Monitor, assess, and document resident conditions and responses to care
- Process physician orders accurately and maintain complete resident documentation
- Assist with admissions, transfers, and discharge processes
- Perform clinical procedures including wound care, catheter care, tube feedings, and specimen collection
- Monitor and record vital signs
- Communicate changes in resident condition to physicians and nursing leadership
- Participate in interdisciplinary care planning and physician rounds
- Educate residents and families regarding care plans and health needs
- Ensure compliance with infection control, HIPAA, and facility safety standards
Requirements
- Active Massachusetts LPN license required
- Graduation from an accredited LPN program required
- Current BLS/CPR certification required
- Long Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred
- Strong clinical assessment, medication administration, and documentation skills
- Ability to supervise nursing assistants effectively
- Strong communication and interpersonal skills
- Ability to work flexible shifts including weekends and holidays
- Ability to work independently in a fast-paced long-term care environment
