Job Title: LPN - Long Term Care

Location: Falmouth, MA

Type: Travel Contract | 13 Weeks

Shift & Schedule: Mixed Days & Evenings | 8-Hour Shifts | 40 Hours/Week | Every Other Weekend & Holiday

Position Overview:
Navitas Healthcare, LLC is seeking LPN - Long Term Care for an exciting travel opportunity in Falmouth, MA.

Responsibilities

• Provide direct nursing care to residents according to individualized care plans
• Supervise and support nursing assistants during assigned shifts
• Administer medications and treatments as prescribed
• Monitor, assess, and document resident conditions and responses to care
• Process physician orders accurately and maintain complete resident documentation
• Assist with admissions, transfers, and discharge processes
• Perform clinical procedures including wound care, catheter care, tube feedings, and specimen collection
• Monitor and record vital signs
• Communicate changes in resident condition to physicians and nursing leadership
• Participate in interdisciplinary care planning and physician rounds
• Educate residents and families regarding care plans and health needs
• Ensure compliance with infection control, HIPAA, and facility safety standards

Requirements

• Active Massachusetts LPN license required
• Graduation from an accredited LPN program required
• Current BLS/CPR certification required
• Long Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred
• Strong clinical assessment, medication administration, and documentation skills
• Ability to supervise nursing assistants effectively
• Strong communication and interpersonal skills
• Ability to work flexible shifts including weekends and holidays
• Ability to work independently in a fast-paced long-term care environment
