Case Coordinator

Hope Home Health Care is looking for one organized, dependable professional with home care experience to join our office team. You'll work alongside our other team members and Director of Nursing to keep operations running smoothly. This is a hands-on role for someone who can wear multiple hats—coordinating active cases, keeping us compliant, and handling day-to-day clerical work. What you'll do: Coordinate ongoing client care once cases are active, making sure everything runs smoothly Monitor caregiver clock-ins and follow up on any missed or late punches Coordinate caregivers, fill shifts, and keep all records accurate and up to date Serve as the day-to-day contact for clients and families, resolving concerns quickly Oversee agency compliance with Ohio and federal regulations, including credentialing and audit readiness Maintain policies and track staff and agency renewals Handle front-office tasks: answering calls, filing, data entry, and supply management Work closely with the Director of Nursing and office team to support quality care Keep all client and staff information confidential What we're looking for: Experience in home care (required) Strong organization and attention to detail—able to juggle several responsibilities at once Clear, professional communication Comfort with computers and scheduling software A reliable, self-directed, team-first attitude #J-18808-Ljbffr Hope Home Health
