This is a direct placement opportunity, offering you the chance to step into a key role with a leading regional healthcare provider. In this vital position, you’ll focus on delivering compassionate care and ensuring the highest standards in end-of-life care for patients and their families.What You’ll Do:Provide nursing services in accordance with the states Nurse Practice Act, facility policies, and Medicare guidelines.Review client referrals and respond appropriately, collaborating with clinical supervisors to determine eligibility for Hospice services.Conduct comprehensive assessments, including OASIS data, within 24 hours of referral, unless other arrangements are made.Document findings, plans, and client progress in accordance with agency guidelines.Develop and coordinate a comprehensive plan of care, collaborating with physicians, therapists, social services, and other healthcare professionals.Provide on-call support and collaborate with a multidisciplinary team to ensure effective Hospice services.Ensure a safe environment for clients and coworkers, adhering to infection control practices and safety procedures.Your Background:Education: Graduate of an approved school of professional nursing. A Baccalaureate degree is preferred.Experience: Minimum of 2 years of nursing experience, with Hospice experience preferred.Licensure: Current RN license in the state of Ohio. Valid drivers license with an insured automobile in good repair.Skills: Strong communication, organizational, and time management skills.
