Home-Based Hospice RN Case Manager - Caring Impact
UnitedHealth Group
UnitedHealth Group in partnership with Amedisys is seeking an RN Case Manager to coordinate compassionate hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that meet each patient’s needs.
You will perform patient assessments, collaborate with the care team, and ensure accurate records while guiding families and caregivers. The role includes on-call responsibilities and a focus on personalized care in the home setting.
