Optum is seeking a Registered Nurse Case Manager for Elite Hospice to deliver the Plan of Care for hospice patients. You will evaluate performance, ensure quality care, and support hospice goals within the patients' homes and communities. The role involves initial nursing assessments within 48 hours of referral, ongoing reassessments, plan of care initiation, and collaboration with families, physicians, and the IDG to ensure comprehensive patient care. #J-18808-Ljbffr
