Acute Care at Home RN & Transitional Case Manager

ST BERNARDS MEDICAL CENTER is seeking a Registered Nurse for Acute Care at Home & Transitional Care in Jonesboro, AR. The role includes hospital-level nursing in home settings, coordination of diagnostics, devices, and therapies, and a 30-day post-discharge follow-up to prevent readmissions.
