Hospice RN Case Manager - PRN (Beaumont, TX)

Optum

At Home Hospice in Beaumont, TX is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will conduct initial evaluations, collaborate with the care team, and implement care plans addressing physical, psychosocial, emotional, and spiritual needs. The role requires current Texas RN licensure, 1+ year of clinical experience, and the ability to travel within a 50-mile radius.
