Full Time Hospice Registered Nurse (RN) Case Manager

Full-Time RN Case Manager Make a Difference Where It Matters Most. Best Hospice Care of Texas is looking for a compassionate and dependable Full-Time RN Case Manager to join our team! As an RN Case Manager, you’ll manage a full patient caseload while working closely with patients, families, physicians, and our interdisciplinary team to provide compassionate, individualized hospice care. What You’ll Do: Manage and coordinate care for a full patient caseload Complete skilled nursing visits and ongoing patient assessments Develop and update individualized plans of care Provide education, guidance, and support to patients and families Collaborate with physicians and the interdisciplinary hospice team Maintain timely and accurate clinical documentation Participate in IDT meetings and on-call rotation as assigned Coordinate changes in patient condition and ensure continuity of care What We’re Looking For: Current Texas RN license Compassionate, dependable, and organized Strong clinical assessment and communication skills Ability to independently manage and prioritize a full caseload Hospice or home health experience preferred If you’re looking for meaningful nursing work where you can build relationships with patients and families while being supported by a close-knit team, we’d love to meet you!
