Home-Based Hospice RN & Case Manager
Gentiva Hospice
Gentiva Hospice in Georgia is seeking a dedicated Hospice RN / RN Case Manager to deliver and document home-based nursing care under individualized plans of care. You will assess patients, coordinate with physicians and IDG members, and provide compassionate education to families.
The role requires an active Georgia RN license, CPR, and at least 1 year of hospice/home health experience. You will join a team committed to dignity, comfort, and quality end-of-life care.
