Identify and initiate referrals for social service programs. Manage the care plan throughout the continuum of care as a single point of contact. Communicate with all stakeholders to ensure quality coordinated care. Advocate for patients and families to ensure their needs are represented. Requirements Education Current unrestricted Texas RN license required. Experience 2+ years of experience in a community health or healthcare setting required. Licenses & Certifications Current unrestricted Texas RN license required. Valid driver's license and access to reliable transportation required.
