Job Description
Title: Registered Nurse - Home Health

Schedule: 8 hours per shift

Assignment Details:
- Contract Length: 11 weeks
- Guaranteed Hours: 40 per week

Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- CHPN certification (Preferred)

Roles & Responsibilities:
- Assess patients in home health and hospice settings to identify their needs.
- Develop, document, and evaluate the Plan of Care to ensure consistent quality of care.
- Educate patients and families on care procedures, medication administration, and home safety.
- Supervise and provide clinical direction to support staff.
- Monitor pain management techniques and educate patients and families accordingly.
- Maintain up-to-date patient records and ensure compliance with third-party payer requirements.

Compliance / Notes:
- Perform on-call responsibilities as assigned.
- Attend necessary interdisciplinary team meetings.
