Home Health, RN- Full Time (CovenantCare)

Job Summary The Home Health RN plans, organizes, and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their and communities. Responsibilities Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illnesses. Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan of care. Education & Experience Required Degree: Associate’s Degree in Nursing. Preferred Degree: Bachelor’s Degree in Nursing. Certificate(s): RN License and CPR certification, if required by state. Experience: 1+ Years. Knowledge, Skills & Abilities Knowledge of home health practices, policies and procedures. Ability to communicate effectively with patients and healthcare professionals. Ability to perform thorough and accurate physical assessments to identify patient needs and health issues. Knowledge of privacy regulations and maintaining patient confidentiality. Ability to analyze complex patient data and clinical information to make informed decisions. Physical Requirements Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard. Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs. Ability to operate various equipment based on department. Ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms). Visual acuity requirements: Near visual acuity at approximately 20 inches or less. Far visual acuity at 20 feet or more. Peripheral vision to observe an area that can be seen up and down or to right or left while eyes are fixed on a given point. Compensation Compensation Pay Range: $35.09 - $49.33 per hour. A reasonable estimate of the pay range for this position is $35.09 - $49.33 per hour, considering job-related qualifications, experience, skills, education, geographic location, and internal and external equity.
