How you'll make an impact in this role

• Drive participant enrollment for the Program of All-Inclusive Care for the Elderly (PACE) by managing the full enrollment process while maintaining program capacity goals.
• Build and maintain strategic partnerships with hospitals, home health agencies, hospice providers, and community organizations to generate referrals and support participant transitions.
• Lead community outreach, educational presentations, and special events to increase program awareness, strengthen community engagement, and expand referral networks.
• Monitor enrollment metrics, maintain accurate reporting and statistical data, and identify opportunities to improve participant acquisition and retention.

What minimum requirements you'll need

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required
