Qualifications / Job Requirements

• Active Registered Nurse (RN) license in the state of New Jersey
  
• Minimum 1 year of nursing experience; Patient Care Coordinator, clinical coordinator, or related nursing role preferred
  
• Strong communication, clinical documentation, and organizational skills
  
• Ability to work collaboratively in a multidisciplinary healthcare team
  
• Experience in specialties such as Thoracic Surgery, Surgical Pathology, Sub-Acute care, Pediatric PACU, or Pediatric OR is a plus
  

Key Responsibilities

• Coordinate patient care activities to ensure seamless transitions across departments and services
  
• Advocate for patient needs and facilitate clear communication between patients, families, and healthcare providers
  
• Monitor patient progress, update care plans, and support clinical decision-making
  
• Lead and support discharge planning, follow-up care, and referral coordination
  
• Maintain accurate nursing documentation and uphold best practices in patient safety and quality care
