Role Description The Direct Care Worker is a full-time, on-site role based in Bucks County, PA, providing hands-on support to clients in their homes. Day-to-day responsibilities include assisting with personal care activities such as bathing, grooming, dressing, and toileting, as well as helping with mobility, transfers, and safe use of assistive devices. The Direct Care Worker supports clients with light housekeeping, meal preparation, medication reminders, and errands to promote a safe and comfortable living environment. The role also involves monitoring clients’ well-being, documenting observations, and promptly reporting changes in condition to supervisors or appropriate care team members. Direct Care Workers are expected to build respectful relationships, follow care plans and agency policies, and uphold client confidentiality and professional ethics at all times. Qualifications Ability to provide compassionate, person-centered care, including assistance with personal hygiene, mobility, and activities of daily living. Skills in maintaining a safe environment, including basic home safety awareness, infection control practices, and proper use of equipment. Strong communication and interpersonal skills for interacting with clients, families, and care team members in a respectful, professional manner. Reliable time management and organizational skills to follow care plans, complete tasks, and document services accurately. Physical ability to perform care duties such as lifting, transferring, standing, and walking for extended periods. High school diploma or equivalent preferred; completion of caregiver, home health aide, or related training is beneficial. Previous experience in home care, assisted living, or direct support roles is advantageous but not always required.
