Responsibilities:

• Develop and implement social services programs that cater to the needs of our residents, including counseling, case management, and discharge planning

• Manage a caseload of residents, providing individualized support and guidance to ensure their social and emotional well-being

• Collaborate with healthcare professionals to develop and implement comprehensive care plans that address the physical, emotional, and social needs of our residents

• Coordinate with community resources and outside agencies to provide additional support services to residents

• Participate in multi-disciplinary care conferences to ensure seamless care and communication among team members

• Recruit, train, and supervise social services staff to ensure excellent service delivery

• Monitor and evaluate program effectiveness, making recommendations for improvements

• Develop and maintain relationships with community partners and stakeholders

Requirements:

• A Master's Degree in Social Work

• Bilingual Spanish - preferred

• Experience managing Social Service Aides

• Experience working with residents in a skilled nursing facility with behavioral/psychiatric needs or diagnoses
