Home Health Registered Nurse | Northeast Central Indiana Help patients receive care where they feel most comfortable Care happens differently in the home. As a Home Health Registered Nurse with Caretenders, part of LHC Group and the Optum family of businesses, you'll help patients receive skilled nursing care while remaining in the place they know best. You'll build meaningful relationships with patients and families, coordinate care with physicians and interdisciplinary team members, and play an important role in helping patients achieve their health goals. For experienced home health clinicians, this is an opportunity to practice with autonomy while making a direct impact on the lives of patients throughout the Greater Indianapolis area. What you can expect In this role, you will provide and direct nursing care in the home as ordered by the physician and in accordance with applicable regulations and agency policies. You will: Complete initial and comprehensive nursing assessments Develop, implement and revise patient plans of care in collaboration with physicians and other healthcare professionals Perform OASIS assessments and medication reconciliation Coordinate care with patients, families, physicians and interdisciplinary team members Provide high quality clinical care within the scope of practice and infection control standards Supervise home health aides and licensed practical or vocational nurses in accordance with state and federal regulations Ensure timely and accurate documentation Support patient safety initiatives and positive clinical outcomes Participate in orientation and mentoring activities as needed Participate in on call and weekend rotation as needed to support patient care Role details Position: Home Health Registered Nurse Coverage area: Northeast Central Indiana, including Wabash, Grant, Huntington, and Blackford Counties. Employment type: Full-time Full-time schedule: Monday through Friday Compensation structure: Per Visit (PPV) Sign-on bonus: $10,000 for eligible full-time external hires Benefits: Available for eligible employees Required qualifications Current and unrestricted RN licensure in state of practice 1+ years of Home Health experience Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation Current CPR Certification Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client Preferred qualifications Ability to work independently Solid communication, writing, and organizational skills Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.27 to $50.48 per hour based on full-time employment.
