Key Responsibilities:

• Conduct patient assessments, diagnose medical conditions, and develop appropriate care plans.

• Administer prescribed medications and treatments.
• Maintain accurate and up-to-date patient records.

Qualifications:

• Nurse Practitioner (NP) degree
• ANCC Certification
• Valid NY Medicaid Provider status
• Previous experience in home health preferred
• Strong communication and interpersonal skills

Working Conditions:
