Weekend On-Call RN Case Manager for Hospice & Home Health

HARBOR HEALTHCARE SYSTEM

Harbor Healthcare System in Lufkin, Texas seeks a Weekend On-Call Registered Nurse (RN) Case Manager to join our hospice/home health team. The role includes admissions, discharges, pronouncements, and assisting the Triage Nurse, with some assessments required.

A Texas RN license is required; 1+ year RN experience is preferred, and travel with reliable transportation is expected. We offer healthcare benefits (medical, dental, vision), 401(k), PTO, and holiday pay, with weekend on-call hours
