Occupational Therapist (OT) – PRN Home Health Phoenix Metro Area | Flexible Schedule | Competitive Per Visit Pay Help Patients Regain Independence Where It Matters Most Brighter Days Home Health is seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team on a PRN basis. We are a clinician‑led agency that believes therapy should be patient‑focused, flexible, and rewarding — not rushed or overloaded. Responsibilities: Perform occupational therapy evaluations and ongoing reassessments in accordance with physician orders. Develop individualized treatment plans focused on improving safety, independence, and quality of life. Provide interventions related to activities of daily living (ADLs), mobility, upper extremity function, cognitive skills, energy conservation, and home safety. Educate patients and caregivers on adaptive equipment, fall prevention, and functional independence. Accurately complete documentation in compliance with agency policies and Medicare guidelines. Collaborate with nursing staff, therapists, physicians, and interdisciplinary team members. Participate in care coordination and case conferences as needed. Monitor patient progress and communicate changes appropriately. Qualifications: Current Arizona Occupational Therapist license in good standing. Graduate of an accredited Occupational Therapy program. Minimum 1 year OT experience preferred. Home health experience preferred. Current CPR certification. Reliable transportation, valid driver’s license, and current auto insurance. Strong communication, organizational, and documentation skills. Ability to work independently and efficiently.
