Family-Centered Early Intervention Coordinator

Care Lync

Care Lync is seeking a Supports Coordinator for Early Intervention Services to provide ongoing service coordination for children birth to age 6 with developmental delays or disabilities and their families. You will help families access services, understand rights, and receive coordinated supports to promote child development. The role covers coordinating evaluations, IFSP/FSR planning, multidisciplinary participation, and ensuring documentation and compliance across Medicaid and IDEA Part C, #J-18808-Ljbffr Care Lync
