Community Health Navigator

Cincinnati Children's Office of Academic Affairs and Career Development

Cincinnati Children’s Cradle Cincinnati program is seeking a full-time Care Coordinator to support care coordination activities on the South Campus. The role emphasizes documentation, scheduling, and communication with families and the care team.

Ideal candidates have a high school diploma and at least 2 years of related experience; Ohio CHW certification is preferred. This is a regular, 40-hour-per-week position with 1.0 FTE in a day shift.
