Field RN - Home Health

Field RN Poughkeepsie Location

Are you a Registered Nurse looking for an opportunity to make a meaningful impact in the community? We're seeking a dedicated Field RN to provide home care support across Dutchess and Putnam Counties.

This role focuses on supervising and educating Home Health Aides, conducting patient assessments, evaluating aide skills, completing documentation and chart audits, and ensuring quality care for patients in their homes.

Responsibilities

• Conduct home care field visits and patient assessments.
• Teach and supervise Home Health Aides in the field.
• Conduct HHA evaluations of skills learned in the classroom and in live patient-care situations.
• Provide in-service education and training to Home Health Aides.
• Perform chart audits and ensure accurate documentation.
• Provide field supervision and support to Home Health Aides.
• Monitor patient care and identify changes in patient needs.
• Maintain accurate and timely clinical documentation.
• Communicate effectively with patients, families, aides, and healthcare professionals.
• Support quality care and compliance with applicable home health requirements.

Qualifications

• Active New York State RN license required.
• Associate's or bachelor's degree in nursing required.
• Minimum 1 year of nursing experience required.
• Home health care experience preferred.
• Strong computer skills required.
• Excellent verbal and written communication skills.
• Strong organizational and time-management skills.
• Ability to work independently in a fast-paced environment.
• Bilingual Spanish preferred.
• Valid license to practice nursing in New York required.

Important Details

• Work Structure: Mix of field and office work
• Schedule: 2 office days + 3 field days; some weeks may be 3 office days + 2 field days based on operational needs
• Coverage Area: Dutchess & Putnam Counties
• Salary: $80,000$90,000 per year
• Mileage: Mileage reimbursement provided
