Medical Social Worker- Home Health

Responsibilities:

• Completes assessments of client’s psychological, social and medical care needs and coping mechanisms
• Assesses the client’s home situation and financial resources available to meet the client’s medical and social needs
• Participates in the development and revisions to the client’s Plan of Care
• Works with the client and family members in meeting the client’s social and medical needs
• Maintains and submits accurate, timely, and complete documentation in accordance with agency policies
• Participates in in-service programs, case conferences, and discharge planning

Qualifications:

• Graduate of an accredited college with a Master of Social Work degree or equivalent
• Currently licensed in the State of California
• Completion of a minimum of one year of supervised work experience or the
  equivalent work experience in the field of psychology or sociology
• Experience in a home health care setting preferred

Compensation

• The anticipated pay range for this position is $40.00 – $50.00 per hour. Actual compensation may vary based on experience, education, certifications, skills, geographic location, and internal equity.

This position may also be eligible for overtime, shift differentials, bonuses, or other incentive compensation, where applicable.
