The facility offers a comprehensive range of on-site and outreach services, ensuring providers have the resources needed to deliver high-quality care to a diverse patient population.

Opportunity:
Physician – Internal Medicine

Job Details:

• Join a collaborative care team consisting of 1-2 Family Medicine physicians and 2-3 Nurse Practitioners in a full-time, 1.0 FTE position
• 36 patient contact hours per week with clinic hours Monday through Friday, 8:00 AM – 12:00 PM and 1:00 PM – 5:00 PM
• Average of 16-20 clinic patients per day
• Limited call coverage consisting of phone call only (nursing home questions and after-hours critical lab call backs) on a 1:4 rotation with other local providers
• Electronic Medical Record system: Epic
• On-site services include Physical Therapy, Diabetes Educator, Weight Loss (Non-Surgical), Pathology/Lab, Radiology/Imaging Services, ultrasound, and mammogram
• Outreach services include Mental/Behavioral Health, Podiatry, Orthopedic Medicine, General Surgery, and OB/GYN
• Optional inpatient rounding available at a local hospital located within a mile of the clinic, with nursing home rounds included
• Board Certified or Board Eligible in Internal Medicine required
