We're looking for an entrepreneurial Physical Therapist or Occupational Therapist to become a Prava Practice Owner serving a premier senior living community portfolio in Northern California, including Independent Living, Assisted Living, and Memory Care.

WHAT YOU'LL OWN

As a Prava Practice Owner, you'll be responsible for the clinical vision, community relationship, and day-to-day leadership of your practice—with Prava alongside you as an operating partner.

You'll:

• Establish yourself as the trusted clinical leader within the community.

• Maintain a strong clinical presence and active caseload.

• Build meaningful relationships with residents, families, community leadership, and care teams.

• Develop a strong partnership with the community operator alongside Prava leadership.

• Understand the operator's priorities and translate them into clinical and operational opportunities.

• Identify opportunities for therapy and early intervention.

• Develop a culture of exceptional clinical care.

• Mentor and develop your therapy team.

• Lead clinical communication and community partnerships.

• Participate in community meetings and interdisciplinary collaboration.

• Help educate community staff about therapy, functional decline, mobility, cognition, safety, and appropriate referral opportunities.

• Monitor clinical quality, resident outcomes, documentation, and program performance.

• Build referral relationships and strengthen the visibility of your practice.

• Identify opportunities to expand services and improve the resident experience.

• Work with Prava leadership to establish goals, review performance, and execute growth strategies.

• Collaborate with Prava on operational, financial, clinical, and growth priorities.

• Represent your practice and Prava with professionalism, integrity, and clinical credibility.
