Licensed Practical Nurse

Delivering high-quality nursing care remotely, the full-time Licensed Practical Nurse will conduct patient assessments, develop care plans, and coordinate with healthcare teams to manage chronic and acute health issues using telehealth technologies. Key responsibilities Conduct thorough patient assessments via telehealth platforms to evaluate health status and identify care needs Develop and implement individualized care plans in collaboration with patients and healthcare teams Monitor patient progress and response to treatment, adjusting care plans as necessary to optimize outcomes Required qualifications Current and valid Licensed Practical Nurse license in the United States Minimum of one year of clinical nursing experience, preferably in outpatient, community health, or telehealth settings Basic proficiency with electronic health record (EHR) systems and telehealth technology Ability to work independently in a remote environment while managing time and priorities efficiently
