Licensed Practical Nurse (LPN) - Long Term Care

Position OverviewAs an LPN in long-term care, you provide ongoing chronic disease management, daily nursing care, and routine assessments for residents in assisted living or nursing facilities.The role emphasizes person-centered care, safety, and coordination with a multidisciplinary team to maintain residents' quality of life.ResponsibilitiesMonitor vital signs, administer medications, and document responses over time.Perform wound care, dressing changes, and routine procedures per care plans.Assess changes in condition and communicate with RNs, physicians, and families.Assist with activities of daily living and rehabilitation activities as directed.Maintain accurate records in the electronic health record and ensure safety protocols.QualificationsEducation: Graduate of an approved practical nursing program.Licensure: Current LPN license in the state of practice.Certifications:CPR/BLSExperience or Skills: 1+ year of long-term care or skilled nursing experience; strong communication with families and team members.ExperienceExperience delivering ongoing care in long-term care settingsExperience with care planning and documentation in an EHRPreferredExperience with wound care or palliative care principlesWork EnvironmentFacility-based long-term care setting with rotating shifts; collaborative team of nurses, CNAs, and therapists.Documentation in the EHR; regular coordination with physicians and family members.
