Description Compensation Range: $23/hr - $26/hr commensurate with experience Benefits We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books. Overview The Care Manager serves as a vital liaison between participants, agency staff, and healthcare providers, ensuring coordinated access to medical, behavioral health, and supportive services. This role supports individuals living with Hepatitis C, those with HIV-related needs, and people at increased risk for infection through outreach, education, rapid testing, care coordination, and navigation to treatment and other essential services. Working within a low-threshold harm reduction environment, including drop-in centers and community outreach settings, the Care Manager conducts intakes, risk assessments, and screenings; provides individual and group prevention education; assists with HIV and Hepatitis C testing; and facilitates connections to medical, psychiatric, social, and emotional support resources. The Care Manager develops trusting relationships with participants, helps reduce barriers to care, coordinates referrals and follow-up services, and promotes overall health, recovery, self-sufficiency, and well-being. Additional responsibilities may include participant recruitment, administering surveys and questionnaires, collecting and entering data, maintaining client records and databases, supporting program events and outreach activities, promoting services, and facilitating educational or support groups. Responsibilities Primary Conduct participant intakes, comprehensive risk assessments, and individualized care planning to identify health needs, assess access to healthcare services, and establish personal goals for care and wellness. Develop and maintain trusting relationships with participants, helping them access appropriate resources and services while supporting their health, recovery, self-sufficiency, and overall well-being. Conduct community engagement activities, including educational workshops, rapid HIV and HCV testing events, and facilitation of linkage-to-care partnerships. Deliver health promotion, health education, harm reduction education, and risk-reduction counseling tailored to the unique needs and circumstances of participants. Promote and support participants in accessing and navigating HIV prevention services, including Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP). Provide rapid HIV and Hepatitis C (HCV) testing, along with pre- and post-test counseling, for participants at elevated risk and coordinate medical appointments, referrals, linkage to care, and follow-up services for those with positive test results. Maintain consistent communication with participants, agency staff, healthcare providers, and multidisciplinary care teams to ensure effective care coordination and service delivery. Assist participants with navigating healthcare and social service systems, including appointment scheduling, accompaniment to appointments when appropriate, communication with providers, and follow-up support. Conduct ongoing participant engagement, monitor progress toward goals, assess outcomes, and perform follow-up activities to support retention in care and services. Participate in clinical case conferences, departmental meetings, clinic huddles, and other team-based care coordination activities as required. Facilitate harm reduction-focused groups that foster education, peer support, skill development, and community engagement within a safe and inclusive environment. Maintain accurate and up-to-date participant records, case files, databases, and program documentation in compliance with agency, funder, and regulatory requirements. Complete all required service documentation and data entry in a timely manner, preferably within two business days of service delivery. Compile and analyse monthly program data and assist with the preparation of monthly, quarterly, and funder reports, as well as presentations for staff and stakeholder meetings. Participate in Hepatitis C funder and stakeholder meetings to provide program updates, share outcomes, and contribute feedback regarding Hepatitis-related services and initiatives. Support the planning, coordination, and implementation of agency-sponsored testing events, outreach initiatives, and public health campaigns. Ensure compliance with all testing protocols, policies, and quality assurance standards, including maintenance of control logs, test inventory, supply ordering, training documentation, and testing records. Secondary Attend meetings as appropriate and related to the scope of services. Participate in quality assurance, quality improvement, staff development, and peer worker development activities as directed. Adhere to workplace policies and procedures, including confidentiality, documentation standards, communication channels, workplace culture, dress code, and conflict resolution. Participate in quality assurance and quality improvement planning project and activities as directed Participates in monthly staff and peer workers development meetings and interventions Requirements High school diploma or General Equivalency Diploma required New York City Department of Health Peer Certificate Training or the ability to obtain certificate through attendance at an AIDS Institute Authorized training center. Good verbal, written, communication and interpersonal skills. Use of computer software --Microsoft Office Suite including Word and Outlook calendaring, and Excel. Frequently remaining in a stationary position, often standing or sitting for prolonged periods Occasionally moving around to additional worksites Occasionally lifting and moving objects up to 25 pounds in ordinary office related tasks.
