Green Key Resources seeks a Care Manager to serve as the primary liaison between members and the Plan, coordinating and reviewing person-centered service plans. The role focuses on health education, ongoing member communication, and ensuring a positive member experience with thorough documentation. Required background in health care or long-term care, RN or MSW, and a bilingual preference.
