Facility Address: 3475 N Saratoga St, Oak Harbor, WA 98277ResponsibilitiesCoordinate appropriate referrals to community and other Government agencies for services.Independently assess the psychosocial functioning and needs to patients and their family members and formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team. Independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diverse backgrounds.Use medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.Implement treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.Independently provide counseling and/or psychotherapy services to individuals, groups, couples, and families. Must practice within the bounds of the license or certification.Provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.Participate in professional peer review case conferences, research studies, or other organizations required.Counsel individuals and families having psychosocial problems related to illness, handicaps, or difficulties that are encountered in the separation from sponsor, adaptation, or stabilization in a new community.Counsel patients on problems such as unsatisfactory relationships between marriage partners or between parents and children, spouse and child abuse, work adjustment, and referrals for human services. Assist the unwed mother with parenthood issues.Aid patients and their families with personal or environmental difficulties that predispose illness or interfere with obtaining maximum benefits from medical care. Work in close collaboration with psychiatrists, medical doctors, other members of the health care team, and community resources to further their understanding of significant social and emotional factors underlying the patient's health problem. In the coordination planning process, help patients and their families through individual or group conferences to understand, accept and follow medical recommendations, and provide services that assist in restoring patients to optimum social and health adjustment within his/her capacity. Use resources such as family and community agencies to assist the patient to resume life in the community or to learn to live with his/her disability.RequirementsDegree/education: Master's degree in Social Work (MSW). Graduate from a School of Social Work full accredited by the Council on Social Work Education (CSWE)Certification: Shall maintain current certification in one of the following: American Heart Association Basic Life Support for Healthcare Providers or American Heart Association Healthcare Provider Course.Experience: Shall be able to demonstrate clinical competency within the past two years in the discipline.Licensure/Registration: Possess and maintain a current, unrestricted license or certification as a Certified Addictions Counselor, Alcoholism and/or Drug Abuse Counselor in any one of the 50 states, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.Other: Provide two letters of recommendation written within the preceding two years from practicing providers, supervisors, or program administrators attesting professional skills, competencies, patient rapport, training abilities, etc. Recommendation letters must include name, title, phone number, date of recommendation, address, and signature of the individual providing.TermsServices shall be provided Monday-Friday, between the hours of 0700 and 1630 hours. Shifts will normally be scheduled for an 8.5-to-9-hour period, to include an uncompensated 30-to-60-minute lunch break, respectively. In no instance will regular services provided be in excess of 80 hours per two-week period.
